Office Policies
We provide services for the following insurance companies and community agencies:

• Blue Cross Blue Shield PPO

• PHCS  out of network
• Greatwest out of network
• Medicare
• Department of Assistive & Rehabilitative Services (DARS; Formerly TRC)
• Mental Health and Mental Retardation (MHMR)

We will also file insurance for you if we are an out-of-network provider on your insurance plan; however, you will be responsible for paying for services at the time of your appointment.

For your convenience, we also accept checks, Visa, and MasterCard.

Please make note that we do have extended hours into the evening, but sessions are done by appointment only.
